The requirements for submittal of the Stormwater Pollution Prevention Plan (SWPPP) are contained in the Checklist (PDF) that is used for reviewing all plans in Elkhart County. The SWPPP Guidance Document (PDF) contains an explanation of each item contained in the checklist. If your construction site is outside Elkhart County, contact the local authorities to find out if there are additional requirements for plan submittal. (Local ordinances outside of Elkhart County may be more stringent than Rule 5 and may use a different review procedure, but they must follow at least the minimum requirements of the rule.) Beyond submitting the proper paperwork, requirements of the rule include the following:
FormsChecklist (PDF): When submitting a SWPPP to the SWCD, all items in this checklist must be addressed adequately in order for the plan to be approved. Guidance Document (PDF): This gives a detailed explanation about each required item in the checklist. Application Form (PDF): This form must be submitted to the SWCD along with the filing fee and the SWPPP. Stormwater Clearance Renewal Form (PDF): This form must be submitted to the SWCD when the stormwater clearance needs to be renewed, along with the appropriate filing fee. IDEM's Notice of Intent (NOI) Form (PDF): Submit this to the state office along with all required documentation and fees, once local approval has been granted. You must wait 48 hours from the postmarked date of the NOI submittal to begin any land-disturbing construction activities (including site preparation). IDEM's Notice of Termination (NOT) Form (Form 51514 under Stormwater & Wetlands): Upon completion of the construction project, removal of all temporary erosion and sediment control measures, and stabilization of the entire site, the owner must submit this notice. The project will be subject to renewal fees until it is terminated. Self-Inspection Report Cover Sheet (PDF): This should be used by site owners or their representatives as a cover sheet for their self-inspection reports. The questions listed are examples of the kind of things the inspector should look for. Self-Inspection Report Form (PDF): This form should be used for regular site inspections, to occur weekly and after every half-inch rain event. Spaces are available to fill in the required information based on the site conditions and BMPs for each particular site.
